ASSUMPTION AND ACKNOWLEDGMENT OF RISKS AND RELEASE OF LIABILITY AGREEMENT

In consideration of being allowed to participate in any way for the United States Adult Soccer Association (USASA), its affiliates, leagues, and member teams, and related events and activities, I acknowledge and agree that:

1) The risk of injury or illness from soccer activities is significant, including the potential for permanent paralysis and death. While rules, equipment, and discipline may reduce this risk, serious injury or illness may occur.

2) I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the Releasees, and assume full responsibility for my participation.

3) I agree to comply with all stated and customary terms and conditions for participation.

4) I release, indemnify, and hold harmless USASA, NPSL, District Elite FC, their officers, officials, agents, sponsors, and affiliates from any and all injury, disability, death, or loss or damage to person or property arising from participation, whether caused by negligence or otherwise, to the fullest extent permitted by law.
