District Elite FC Tryout Waiver and Release

Health and Participation:

I understand that physical activity carries inherent risks, including falls, muscle or ligament injuries, circulatory or heart issues, and other potential health complications.

I confirm that my health is adequate for participating in strenuous and vigorous physical activity, and I have consulted with a physician if needed.

Insurance and Medical Coverage:

I understand that District Elite FC does not provide insurance to cover medical expenses for tryout participants. I confirm that I have my own medical insurance coverage.

Liability Release:

I voluntarily assume all risks associated with my participation in the tryout and release District Elite FC, its employees, coaches, agents, officers, and assigns from any claims, damages, or liabilities that may arise from my participation.

Acknowledgment of Responsibility:

I confirm that I have had a recent sports physical and am medically fit to participate.

I understand I have the right to withdraw from the tryout at any time for any reason.

I accept responsibility to report any injury, distress, pre-existing condition, or other concerns to the coach or athletic trainer during the tryout.
